CoxHealth Medicare Advantage (HMO) prescription drug formulary or “Drug list” is a list of prescription drugs covered by us. The drug list is created by the plan in coordination with doctors and pharmacists. Your list includes brand name and generic name drugs.

Formulary Updates
Step Therapy
In some cases, CoxHealth Medicare Advantage (HMO) requires you to first try certain drugs to treat your medical condition before we will cover another drug for that condition. This is called Step Therapy (ST). For example, if Drug A and Drug B both treat your medical condition, we may not cover Drug B unless you try Drug A first. If Drug A does not work for you, we will then cover Drug B. For more details on Step Therapy (ST), click here for 2026.
Prior Authorization
CoxHealth Medicare Advantage (HMO) requires you or your physician to get Prior Authorization (PA) for certain drugs. If there is a PA requirement, this means that you will need to get approval from CoxHealth Medicare Advantage (HMO) before you fill your prescription(s).
