When you enroll in CoxHealth Medicare Advantage, you can expect to receive a phone call from Customer Service welcoming you to your new plan. We will verify your identity and confirm that we have received your enrollment. Then, we will verify your address to ensure you will receive any plan mailings, including a welcome packet with plan information and your Member ID Card. We will also confirm that we have your correct primary care provider on file to ensure continuity of care, and we will check to see if you have any questions about your new plan. Our goal is to make sure you are off to a great start with CoxHealth Medicare Advantage!

We encourage you to complete this Health Risk Assessment (HRA) questionnaire within 90 days of your effective date of coverage to get connected with resources and wellness activities based on your specific needs.
